Congratulations on making the commitment to participate in North Arlington. High School Athletic Program. Our dedicated coaching staff is committed to each and every student/athlete. We strive to guide each student/athlete to reach their greatest potential both in the classroom and on the field of play.
 
Please pay special attention to all the enclosed information.  It is imperative that all paperwork is accurately completed, signed, and submitted to the Physical Education teachers or Athletic Office to ensure approval and eligibility for sports.  Athletic Physicals must be current within 365 days. Each physical must be conducted by a physician trained the NJDOE Cardiac Module. If you are not sure of the date of your last physical contact the Athletic Office  (201-991-6800 ext. 2000). 
 
Scenario 1: Student does not have a current physical and needs to register for a sport.
You need to register through the On-line Registration. You would need to print a copy of the Preparticipation Physical Evaluation Form. The form must be filled out by your physician and returned to the school by the posted due date.                                                                                                                                
 
Scenario 2: Student has a current physical and needs to register for a sport. 
You need to register through the  On-line Registration. You will also need to print a copy of the Health History Update Questionnaire. The form must be filled out by the parent and student and returned to the school by the posted due date. 
 
Scenario 3: Student wants to obtain a physical from the school doctor and register for a sport.  
You need to register for the fall season through On-line Registration. You will also need to print a copy of the Preparticipation Physical Evaluation Form. Fill out the History Form and return to school by the posted due date, prior to the scheduled doctor visit.  
 
Scenario 4: Student wants to obtain a physical from the school doctor and is not registering for a sport.
You need to register for the doctor visit by completing the Doctor Registration Form. You will also need to print a copy of the Preparticipation Physical Evaluation Form. Fill out the History Form and return to school by the posted due date, prior to the scheduled doctor visit.    
 
Scenario 5: Student wants to be a team manager or bookkeeper. 
You need to register through the Team Manager On-line Registration.
